Please explain this gag cartoon

I was a fan of MAD Magazine in my youth, and recently picked up an old
book of collected cartoons by Sergio Aragones, the guy who drew all of
those "Mad Marginals." I always liked his mostly wordless gag
cartoons.

But here's one in the book that absolutely mystifies me:
